Phyllis A. Perry

Phyllis A Perry, age 86, of Norwalk, passed away Friday, August 28, 2026, at Brethren Care Village in Ashland.
She was born August 28, 1940, in Lorain, OH to the late Peter and Carol Sklarek. She was a 1958 graduate of Lorain High School, and a 1961 graduate of the Providence School of Nursing. She worked 32 plus years as a Registered Nurse at Fisher-Titus Medical Center. She enjoyed caring for the elderly and learned so much from them, their suffering and stories. She was an avid dancer of Polish music, entertaining and traveling with her family and friends. She especially enjoyed attending her grandchildren’s activities and spending time with them. She had an affinity for praying to the Blessed Mother.
She is survived by her son, Rick (Heather) Perry and their children, Isaac (Corynne) Perry, Alexa (Aaron) Bristol, Mitchell Perry and Jared Perry; daughter, Kathleen (Daniel) Holtz and their children, Brooke and Evan; daughter-in-law, Wanda Perry; twin sister, Patricia Kuzak; nieces and nephews.
She was preceded in death by her son, Dale Perry; husbands, Ray, Russ and Richard; brother, Ken (Mary) Sklarek; and her brother-in-law, Ronald Kuzak.
A funeral Mass will be held Friday, September 4, 2026, at 11:00 A.M. at St. Paul Catholic Church, 91 E. Main Street, Norwalk. A luncheon will follow and private burial will be held at St. Paul Cemetery.
Arrangements entrusted to the Evans Funeral Home, 314 E. Main Street, Norwalk.
